ACA/NY – Advance Care Alliance of New York (ACA/NY) is a 501(c)(3) non-profit organization that is one of seven New York State Care Coordination Organizations (CCO) designated by the New York State Department of Health and the Office for People With Developmental Disabilities (OPWDD) to offer comprehensive care coordination to people with intellectual and/or developmental disabilities (I/DD). ACA/NY supports over 25,000 people in leading active, healthy and fulfilling lives throughout New York City, Long Island and the lower Hudson Valley region (Orange, Rockland and Westchester Counties).
