Care Coordination & Health Comm provides supports through an experienced and knowledgeable service coordinator who advocates for the family and helps them to access needed services Supports may include in-home and out-of-home respite, crisis intervention, counseling and access to recreation activities. The Committee helps access general medical and clinical services which might include PT, OT, Speech Therapy, physiatry, specialized dentistry, durable medical equipment and social work.
